If you are ready to apply for 501(c)(3) tax exempt status for your charity, or your proposed charity, please proceed by doing one of the following:
- Call us at (954) 560 7538 to request our Information Questionnaire. You will need to complete this form and return it to us so that we can get the process started. We will also provide you with our Service Agreement which outlines the terms and conditions of our services. We can send the documentation to you via email, fax or mail. Email is preferred.

- You can get the process started by filling out the simple Online Questionnaire below. The Online Questionnaire seeks to obtain some preliminary information about your charity or proposed charity. Once we receive your communication via the internet, we will then provide you (via email) with further information to get the process going.
